Navigating Molina Healthcare’s "Autism Care Demonstration" and Medi-Cal requirements can feel overwhelming. Use this checklist to ensure your child’s therapy starts as quickly as possible.

1. The Primary Care Physician (PCP) Visit

Before ABA can be authorized, Molina requires a fresh recommendation (within the last 12 months) from your child's pediatrician or a specialist.

What to ask for: A "Referral for Behavioral Health Treatment (BHT)/ABA Services."
Specific Form: Ask your doctor to fill out the Molina Medi-Cal ABA Referral Form.

2. The Diagnostic Evaluation

Molina requires a Comprehensive Diagnostic Evaluation (CDE) or a Psychological Evaluation to confirm the diagnosis.

Keep a Copy: Ensure you have a signed copy of the evaluation that includes the specialist’s license number and NPI.
Need a Diagnosis? If you don't have one yet, your doctor must use the Molina BHT Psychological Evaluation Referral Form (Page 2).

3. The Functional Behavior Assessment (FBA)

Once we have your referral and diagnosis, Golden View Learning Center will request authorization from Molina to perform an FBA.

The Process: A Board Certified Behavior Analyst (BCBA) will observe your child and design a custom treatment plan.
Turnaround: Molina typically reviews these requests within 5-7 business days.

4. Submitting for Ongoing Treatment

After the assessment, we submit the final Prior Authorization (PA) request to Molina. This includes:

The Treatment Plan.
The Physician’s Referral.
The CDE/Diagnosis (F84.0).
